What the ACF’s Focus on Interoperability Means for Community Care

The Administration for Children and Families (ACF) within the Dept. of Health and Human Services, has amplified its commitment to interoperability within the human services ecosystem. This focus on seamless data sharing and cross-sector collaboration has significant implications for Community Care Hubs (CCHs) and the organizations they support. Interoperability represents more than a technological shift—it’s a strategic framework to improve outcomes for individuals and families navigating complex systems of care.

Why Interoperability Matters

Interoperability enables organizations to share, interpret, and act on data across sectors efficiently and securely. It bridges siloed operations, ensuring that information flows freely between providers, social service agencies, and other support systems. This creates a coordinated network that reduces redundancy, improves service quality, and accelerates response times for individuals in need.

ACF’s Interoperability Action Plan outlines a roadmap to enhance data-sharing capabilities across human services by 2027, with data standards expected by the end of 2024 and pilot programs targeted for 2025. We at IMPOWR appreciated the opportunity to provide public comments on this important body of work. While there is a new administration in the White House this year, it is unclear what impact it may have on these efforts. Until decisions are made to change the scope or timelines, organizations continue working diligently to advance these plans.

Implications for Community Care Hubs

Community Care Hubs are uniquely positioned to benefit from ACF’s interoperability efforts. These hubs, which coordinate care across health, social, and community services, rely on information exchange to provide holistic support. Interoperability can:

  • Strengthen Collaborations: Connecting hubs with hospitals and social services ensures comprehensive care.

  • Enable Insights: Timely data access highlights trends and informs proactive interventions.

  • Simplify Compliance: Automated reporting reduces administrative burdens.

  • Promote Equity: Standardized data sharing ensures individuals receive tailored support without falling through systemic cracks.
