The need to make it easier for everyone to provide necessary, sustainable, and effective health-related social needs services (HRSN) to their members!
The shift towards coordination and integration in the Aging space, along with the incorporation of new HRSN services, provides many unique opportunities for partnership. However, these news partnerships are comprised of organizations that have traditionally provided services in different sectors, and while they are all well-versed in doing business in their traditional manner, these new partnerships require new ways of doing business.
These emerging models are all centered around identifying needs and coordinating services across various service providers, collecting data and demonstrating impact, and billing to ensure long-term sustainability. The underlying structure is the same whether it’s called a community care network, social care network, or Triple A (AAA) Network. The best networks are focused on helping to navigate people through referrals and get their needs met, while working together to reduce duplication in services, meeting reporting/contractual requirements, and deepening their relationships within the community. Regardless of the composition of these networks, they all need infrastructure- social care infrastructure. An effective social care infrastructure provides a scalable way to coordinate amongst the various providers allowing them to coordinate referrals, share assessments, align on care plans, document services, and share the right level of information in a secure and compliant manner.
Research shows that including HRSN services within a community is not only better for the patients, but also far cheaper than traditional medical/clinical interventions. This dovetails into the growing movement of value based payment models and contracts for providing for HRSN across the United States. To be successful in the long term, networks must move beyond siloed service provisioning, and into a more complex world with workflows starting with eligibility and ending in generating claims/invoices.This doesn’t happen without a clear vision and the right tools.
The execution of this vision must also incorporate the other great ongoing work at these organizations, which typically include multiple funding streams (grants, state and federally funded financial arrangements, donors, etc.) and workflows. These networks and member organizations don’t need another “band aid” portal for data entry or billing, these networks and their members need a true social care infrastructure. An infrastructure that can support all stakeholders and multiple workflows within and across each network. Without the right infrastructure, even the most well intentioned networks may become another red tape laden system- increasing frustration for the people who most need the care and those caring for them.
