We are proud to announce that IMPOWR has received the nationally recognized Pathways Community HUB Institute® Model Provisional Certification as a Technology Vendor. The PCHI® Model is one of the largest cohorts of standardized and outcome-based community care coordination initiatives in the country. As noted in previous articles, social care networks (SCN) are forming across the nation to deliver health-related social needs, typically coordinated by a community care hub. Although SCNs vary in whom they serve, one thing unites them- the need for quality technology.

 

Since 2018, IMPOWR has been purpose built to support quality social care networks and has strived to partner with organizations focused on meeting the needs of the community. This dedication led IMPOWR to collaborate with PCHI, who for more than 20 years, has helped communities across the United States create sustainable community-based social care networks. The PCHI Model is a proven, evidence-based approach designed to connect individuals with the services and resources they need to achieve better health outcomes. This Model emphasizes a coordinated social care network that identifies risk factors across various domains—such as housing, employment, education, and healthcare—and connects individuals to dedicated community health workers (CHWs) who can guide them through each relevant "Pathways" to address those needs.

The Journey to Certification

Over the past year, IMPOWR went through the rigorous certification process where the platform had to meet numerous requirements from workflow and user experience, to reporting and data security. IMPOWR software not only met the Model’s robust requirements, but made the CHW work more efficient, allowing for excellent management oversight, custom dashboards, and PCHI reports built in to make sending data back to PCHI take minutes each quarter instead of days. In

addition, the billing process became a breeze and is designed to support braided funding streams – no more having to copy and paste across multiple programs or excel sheets to get billing to the right organization- reduce delays in getting funding. Being certified by PCHI demonstrates our commitment to the highest standards in community care coordination.
